First Aid Immunology — Flashcards for the High-Yield Small Chapter

Immunology is a small chapter with disproportionate Step 1 weight. Cytokines, HLA types, immunodeficiencies. Every fact matters.

The First Aid immunology chapter is roughly 40 pages — small compared to pharmacology or pathology, but disproportionately tested on Step 1. NBME and USMLE-Rx forms consistently include 5–8 immunology-specific vignettes: baby with recurrent Neisseria infections (terminal complement deficiency), boy with eczema + thrombocytopenia + recurrent infections (Wiskott-Aldrich), acute transplant rejection mechanism (T-cell-mediated).

The chapter covers innate vs adaptive immunity, cytokines and interleukins, HLA associations, T-cell and B-cell activation, immunoglobulin isotypes, complement pathway, hypersensitivity reactions (types I–IV), primary immunodeficiencies (B-cell, T-cell, combined, phagocyte, complement), and transplant immunology. It's dense but well-structured.

The cytokine table — the hardest single page

First Aid's cytokine table has ~15 interleukins plus IFN-α/β/γ, TGF-β, TNF-α. Each has a source cell and a target/function. This is pure memorization. Card each individually:

  • IL-1 — macrophages → fever, acute inflammation
  • IL-2 — Th cells → T-cell proliferation
  • IL-3 — T cells → bone marrow stem cells (like GM-CSF)
  • IL-4 — Th2 → B-cell class-switching to IgE + IgG
  • IL-5 — Th2 → B-cell class-switching to IgA + eosinophil activation
  • IL-6 — macrophages/Th2 → acute-phase reactants + fever
  • IL-8 — macrophages → neutrophil chemotaxis
  • IL-10 — Treg → anti-inflammatory (dampens Th1)
  • IL-12 — macrophages → Th1 differentiation + NK activation
  • IFN-γ — Th1 → macrophage activation
  • TNF-α — macrophages → cachexia, fever, granuloma maintenance
  • TGF-β — Treg → anti-inflammatory + IgA class-switching

HLA associations — the second-hardest table

HLA-B27 → seronegative spondyloarthropathies (PAIR: Psoriatic, Ankylosing spondylitis, IBD-associated, Reactive). HLA-DR3 → SLE, T1DM, Graves. HLA-DR4 → RA, T1DM. HLA-DQ2/DQ8 → celiac disease. Card each association. High-yield.

Immunodeficiencies — by defective cell type

Group by defect: B-cell defects (Bruton, CVID, hyper-IgM, selective IgA), T-cell defects (DiGeorge, chronic mucocutaneous candidiasis), combined (SCID, ataxia-telangiectasia, Wiskott-Aldrich, hyper-IgE), phagocyte defects (CGD, Chediak-Higashi, LAD), complement defects (C1 esterase inhibitor deficiency, MAC deficiency, C3 deficiency). Each has a signature infection profile and diagnostic test.

Hypersensitivity reactions — the classic type I–IV

Type I (immediate, IgE, anaphylaxis), Type II (antibody-mediated, cytotoxic), Type III (immune complex), Type IV (delayed, T-cell mediated). Every type has classic examples that get tested repeatedly.

How to review immunology decks

Small chapter → shorter review sessions. 15 minutes daily for the first 2 weeks after generation. Then 10 minutes 3× per week for maintenance. Because the material is small and interconnected, spaced repetition works exceptionally well here.

Frequently asked questions

Is immunology tested on Step 2 as well?
